What We Will Discuss During Your Interview

Personal History and Background
  • Current employer’s name, address and dates employed
  • Dependent information including name, SSN, and date of birth
  • Current and *previous spouse(s) information including name, date of birth, SSN, date and location of marriage. *Only previous spouse(s) information needed if marriage lasted at least 10 years or ended in death.
Earnings
  • Year-to-date earnings (estimates are acceptable)
  • Last year’s earnings (estimates are acceptable)
  • Short-term disability, vacation pay, sick pay, salary continuation, bonus, etc. paid after you stopped working (estimates are acceptable)
Work Attempts - (work activity performed after your original date last worked) For example: I initially stopped working March 3, 2013. I returned back to work on May 5, 2013 through June 10, 2013. Work attempt is from May 5, 2013 through June 10, 2013.

Customer Information Center (CIC)

CIC is available to assist you with your questions about the Social Security disability process. Often, your assigned Allsup team member is working behind the scenes making calls to the Social Security Administration or Disability Determination Services to take care of your case. Your assigned Allsup team member may not always be available when you want to call Allsup with questions, but our CIC associates will take care of your needs.

Our associates can check the status of your claim, document your medical updates, and answer questions about forms, the claims process, and any letters you receive from the Social Security Administration or the Disability Determination Services. CIC associates work with your assigned Allsup team member to take care of any issues relating to your claim.

It is important to call CIC if:

  • You experience a life-changing event.
    • Additional diagnosis or hospitalization
    • Financial change (e.g. loss of spouse’s income)
    • Birth of a child
  • You change your address, phone number or physician.
  • You will be unavailable for more than 30 days.
  • You are scheduled for a consultative exam by the Disability Determination Services/Vocational Rehabilitation.
  • You receive a notice of award or denial from the Social Security Administration.
  • You receive a check or a direct deposit from the Department of Treasury.
  • You return to work in any capacity.
  • You experience any changes in Workers’ Compensation or state disability benefits.

Quick Facts About Social Security Disability Insurance

If approved, the Social Security Administration will pay benefits back to your date of entitlement. Your established onset date and/or date of filing determine your date of entitlement. In nearly all cases, the date of entitlement will not be the same as your established onset date.

  • The Social Security Administration determines the onset date of your disability. This may differ from the date you last worked.
  • In addition to your initial claim forms, the Social Security Administration and Disability Determination Services usually request additional information. Requests for information by phone or mail should be referred to our CIC. Contact them immediately to ensure the fastest processing possible.
  • It is common for the Disability Determination Services to schedule a consultative exam. The exam is provided at no cost to you and is ordered when the disability office needs additional or more current information to make a determination. It is very important to attend any consultative exam that is scheduled.
